TITLE: CASE MANAGER

GENERAL DESCRIPTION:

Provide goal directed activities, which will assist the client by locating, coordinating, making linkages and monitoring necessary treatment and services available to the client. The Case Managers activities are community-based and are delivered either in the client’s environment, in community agencies or in our agency. Service will be accessible to the client and their families in the home, school, workplace or where needed.

POSITION REQUIREMENT:

B.A. or B.S Degree in Psychology, Sociology, Social Work, counseling or related human service field. Experience as a case manager or social worker in mental health, social service or substance abuse is preferred. Possess a valid driver’s license, good driving record, appropriate automobile insurance and valid license tags.
